Unifhi is a product of Zaavya LLC. This policy covers www.unifhi.com, the marketing website.

It does not cover a Unifhi implementation. That is governed by the agreement and the business associate agreement signed with your organization.

This website is not a channel for patient data. No protected health information is collected here, and every form on the site says so.

What we collect

When you submit a form

Your name, work email, organization, role, the workflow you selected, and your message. The form also records which page you submitted from and which assessment you requested. It captures any campaign parameters in the link you followed, and the address of the site that referred you.

We use this to respond to your inquiry and to understand which pages produce conversations.

When you visit

Standard log information your browser sends: IP address, browser type and version, the pages you view, the time and date, and time spent on each page.

Third-party services on this site

Google Analytics

We use Google Analytics 4 to understand how the site is used. It sets cookies and sends usage data to Google. You can opt out with Google's browser add-on, or by blocking cookies for this site.

Website visitor identification

We use a visitor identification service provided through Instantly. It identifies the organizations that visit this site, so we can follow up with relevant businesses.

This is business-level identification for sales purposes. It is not individual tracking, and it never involves patient data. If you would rather not be identified this way, block third-party scripts for this site. You can also contact us and we will exclude your organization.

Google Fonts

Typefaces load from Google's font service, which receives your IP address as part of that request.

Where the information goes

Form submissions go to an endpoint hosted on Amazon Web Services in the United States, then to the Zaavya team. We do not sell your information, and we do not share it except as described in this policy or where required by law.

How long we keep it

We retain inquiry information for up to 24 months after our last contact with you, then delete it. Analytics data is retained according to Google Analytics settings.

Your choices

• Analytics: opt out with Google's browser add-on, or block cookies for this site.
• Visitor identification: block third-party scripts, or ask us to exclude your organization.
• Your information: ask us what we hold about you, ask us to correct it, or ask us to delete it.

We use commercially reasonable means to protect the information collected here. No method of transmission or electronic storage is completely secure. We cannot guarantee absolute security.
